The Evelyn Frye Center provides the full continuum of outpatient behavioral services on an urgent, emergent and routine basis.
How?
Have your patients or staff contact our Customer Relations
Department at (615) 385-4090 to schedule appointments.
When your patients contact the Evelyn Frye Center, our staff will inquire about:
- whether they plan to use insurance,
- the nature of the presenting problem,
- any special needs or requests and
- their preference regarding a provider.
Each case is carefully triaged by our professionally trained staff in an effort to get the best clinical/personality fit for each patient.
After the appointment has been scheduled, one of our Business Associates will contact the patient’s insurance company to obtain authorization/certification for services. Once that has been completed, we contact the patient to inform them about the amount of his/her copayment and to reconfirm their appointment date, time and provider information.
Please see the Referral Form at the conclusion of this webpage. This form was developed to facilitate easy referral of your patients to the Evelyn Frye Center. It is not necessary to use this form; however, should you find it useful in making referrals, please have your office contact us by telephone (615)385-4090, fax (615) 385-0138 or e-mail lvowell@efrye.com and we will deliver multiple copies of this form to your office within 24 hours.
